Sir,

With reference to above, we are pleased to place this order for supply of  Cochlear Implant Device with Accessories amounting to Rs.2,467,710.00 as per terms & conditions below. Item details & rates are at Annexure 'A'.The items to be delivered at GMCH, Guwahati.

Terms & Conditions: -

  1. The above price is on FOR destination basis inclusive of all costs including GST, transportation, transit insurance, loading & unloading and any other incidental charge. All admissible taxes as applicable as per existing Govt. rules are liable to be deducted from invoices.
  2. The total ordered quantity must be delivered at sites as indicated at Annexure-‘A’ within 90 days from the date of issue of the purchase order.
  3. The supplied items shall be strictly in conformity with the specifications. If any of the items are found to be inferior or sub standard in quality the same must be replaced with acceptable quality at supplier’s cost within a period of two weeks from date of intimation. In case of failure to replace the defective goods on time or in the event the replaced goods are found inferior or substandard in quality, the supplier will be liable to be blacklisted.
  4. Service Support: Provision of Equipment defects / malfunction within five working days. In case of servicing of speech processor, loaner processor is to be provided by the supplier for the period of servicing.
  5. In case there is delay in delivery beyond the stipulated period as mentioned, there shall be reduction in price @0.5% of the value of delayed goods per week of delay or part thereof subject to a maximum of 10% of the total order value. Once the maximum price reduction is reached, the order will be terminated.
  6. Payments towards the supply of items will be made strictly as per rules of the Tender Inviting Authority. Full payment will be released only after completion of supply of entire ordered quantities within 1 month normally after submission of bills.
  7. Within 10 days of recipt of the P.O.  it shall be required to submit Security Deposit of 5% of the order value renewable to 10 years in the form of bank guarantee from any Indian scheduled bank in favour of “State Health Society, Assam”. However, if the supplier fails to execute the order or fails to perform the services as per agreement, in addition to other penal actions, the bank guarantee shall be encashed and the amount will be forfeited.
  8. All other Terms & Conditions incorporated in the tender referred to above shall be applicable.
