With reference to above and in view of the urgent need of various essential and life saving items for public health care service due to global outbreak of COVID 19, you are requested to supply the following items as per the given terms and conditions:

Terms & Conditions of the Supply order is described below: -
- The above price is on FOR destination basis inclusive of all costs including GST, transportation, transit insurance, loading & unloading and any other incidental charge. All admissible taxes as applicable as per existing Govt. rules are liable to be deducted from invoices.
- The supply should be completed as early as possible after the issue of this P.O. due to COVID-19 emergency.
- The supplied items shall be strictly in conformity with the specifications. If any of the items are found to be inferior or sub standard in quality the same must be replaced with acceptable quality at supplier’s cost within a period of two weeks from date of intimation. In case of failure to replace the defective goods on time or in the event the replaced goods are found inferior or substandard in quality, the supplier will be liable to be blacklisted.
- Payment terms will be 100 % within 30 days from date of receipt and acceptance. Payment will be released on satisfactory completion of supply and installation of entire ordered quantities and against submission of bills and challan bearing stock entry details.
- The Supplier shall have to furnish “Security Deposit” @ 5% of the total order value in the form of “Bank Guarantee” of a scheduled Indian Bank in favour of “State Health Society, Assam” valid upto warranty period, within 10 (ten) days of issue of the purchase order.
- All other Terms & Conditions as specified in the tender reference shall be applicable.
