With reference to above, we are pleased to place this order for supply of Tablet Ivermectin 12mg as per “DETAILS OF ITEMS & PRICES” and “TERMS & CONDITIONS” stated herein below:

Terms & Conditions of the Supply order is described below: -

• 5,00,000 numbers of tablets should be supplied by 7 days from the placement of supply order and supply of remaining 15,00,000 to be completed by next 21 days. However due to COVID-19 pandemic, you are hereby requested kindly to complete the supply within the best possible time.

• Payment will be made within best possible time from receipt and acceptance of materials at destination against submission of bill & receipted challan (in triplicate) with stock entry details.

• Rates of Drugs should not be more than rates approved by GOI(NPPA). If rates are more than NPPA approved rates, then excess amount will be recovered from the manufacturer.

• Any Dispute arising out of the above order shall be referred to the Court within the Guahati Jurisdiction of State of Assam.

• All other Terms & Conditions as specified in the TENDER NO:  NHM-18013/2/2020-PROC-NHM/ECF-145501/23-P, Date:-08.09.2020 shall be applicable.
